I have a contractor who owes me a refund, but will only give it to me if i give him a 5-star review. Which he does not deserve. What are my options?

I am very sorry that this happened to you.
You are doing the right thing to report this.
This freelancer should be sanctioned. Ideally, I would want to see the freelancer banned from Upwork permanently.

Bojan has offered the best solution. In my experience, the moderators are pretty good at follow-through. They're busy, so give him a week.
I don't recommend it, but two can play that game. Leave a public five-star rating, and in the private rating savage him. Now you're stooping to his level, not something I'd do lightly. But, there are times when the only way to fight fire is with fire.
