If you verified a card Upwork will deposit money into that account (typically some cents). You simply need to check your account, find the deposits from Upwork, and then enter the amounts it deposited for the Amounts 1 and 2 fields. I don't know how long it takes for them to make those deposits so it may take some time for them to show.
