Affidavit of Unchanged Status for W-8BEN form - Changed address

Trying to complete W-8BEN form. Have changed address in the last year so not sure how to proceed with the Affidavit of Unchanged Status section. Any help would be great thanks.

Sorry, this doesn't solve my issue. I am trying to complete the Tax Identification Number section. I don't know if I can sign the  Affidavit of Unchanged Status as I have moved address within the last year, so my details have changed

Hi Amy, just stumbled upon this post as I am having the same issue. My address has changed since the listed date so I am not sure if I should sign the affadavit. Did you get any clarity about this?
