Birth date providing Confusion

Hello sir,

today I opened my upwork account. but I forget, did upwork give me any box for providing birthdate? or I have not given any birthdate yet?
I can not remember it now, because I have a confusion about providing the date. I know without providing birthdate we can not open an account but I forgot it I provided it or not.
so, can you check please, my birthdate status is right or have any mistake here?
my birth date is (05- May- 1995)
make sure I provided you my "national ID card " and "Passport" original photo.
please, ensure me.
