Ma'am is sort of a shortened version of Madam.  

And Madame is the French version of Madam.  

Madam is more formal than Ma'am.

Ma'am is used after addressing the Queen of the United Kingdom as "Your Majesty" in a conversation.

 

 

The word "Madam" may be used to address women in general, that is, without any regard for one's marital status.
