I need a manual name review

My license name 'Christoph' and my actual name 'Christopher' do not match. 

I need my profile name to match my checking account name (Christopher) to get paid.

I went through the automated prompts and was brought here:

But the form it tells me to click on goes nowhere (brings me in a circle)...am I missing something? How do I submit a request for a manual review?
