ID does not match profile name when trying to verify identity

I am attempting to upload a photo of my driver's license in order to verify my identity, but it keeps telling me that the name on my ID does not match the name on my profile even though it is the same. I even changed my first name on my profile to my first and middle name because that is how it is listed on the ID but it still didn't work. 

It is a CO license. Line 1 is my last name, Line 2 is my first and then middle name. How can I get the upwork site to accept my ID? It won't even allow me to try again with a new photo. Please help.
