Last Name Problem while applying for payment

I am Kunal, New to Upwork and want to know if you can help me here. My legal name is just Kunal in the bank as well. 

While opening my Upwork Accout, I had to add my Last name so I added Kumar and while verification for verified badge, I Put kunal in there which got verified. 
Now, I am adding my Payment method to direct bank and the verification is not happening. Can you assist what should I put my Upwork name, Name in the attached screenshot fields.

Hi Kunal,

Thanks for reaching out to us. As you only have one name, I suggest that you go to Settings > Contact Info and make changes to your name. You can use 'Kunal' in both the first name and the last name field so the system would know you as Kunal Kunal. After that, you can go and add the payment method with the same name.
