Manual Name Review needed

Hello -

I need a manual name review. My card that is listed is a business account, by which I am the sole owner of. The card has both my name and the business name on it, and it is not recognizing my actual name on the card. Please advise. I can show proof by statement if needed that I am the only authorized signer on the acocunt.
