My Bank Asking for MT103 Form

Hi, I Withdrew 105.01 USD a few days back. The was processed by UpWork the same day but I haven't received anything yet. I contacted my bank regarding this and they are asking for MT103 form so that they can trace the funds. Someone, please tell me how to request MT103 form on UpWork.

Please keep in mind that  It can take between 2 to 5 business days for the funds to be loaded on your bank account.
