I am travelling to US next month for 5 years and I wanted to know should I have to change the location of my UPWORK PROFILE. 

Also should I have to submit any documents?

Yes, you need to change it when arrived. You will be asked for verification after you apply changes. Also tax details should be changed because you will work from US

Which documents will be needed for verification?

Bank statment, house bill, etc. Same as you did before where passed verification.
