No luck adding my tax ID for Canada

Hello, 

I need help with this tax ID section because clearly I am doing something wrong. I have a HST number for my business which is registered under a business name and not my actual name. What are the exact pieces of information I need to get correct in order to have my HST number accepted. Every combination I try fails. 

Any help on this would be geatly appreciated.
