Second identity verification Legit?

Hi, in regard to a "Skill certification assessment" I was asked from  **Edited for Community Guidelines** to do a second identity verification. Attached is a Google Forms where I should put in my E-Mail.

Is this a legit procedure? So an official mail?
I'm asking because I did the verification a week ago.

The following post has nothing to do with the rest of this thread:

If I owned a big website, and I wanted to ask users to participate in identity verification procedures... I would not ask people to use Google sheets or Google Forms, etc.

These are third-party, easily accessible, non-secure tools that are used widely by scammers.
