Should i apply for name change again?

Hi, I was requested for a name change some days ago, so i got an email and ticket to provide your ID card and bank statement, That time my bank statement was not ready now i requested to bank and got my bank statement but the ticket shown the problem solved status, Should i submitt my ducuments on that link or request again for a name change? Thanks
