Assume ANY link outside of the upwork.com domain is a scam. At the very least, it's a violation of the terms of UW prior to signing a contract. I do not know why UW even allow jobs posters to send links. Any link sent should incur a suspension or outright ban. It's trivial to do. At the very least block t.me (Telegram) and other obviously scammer-popular platforms from being linked-to from UW messages sent to freelancers.

Ya Upwork notified me later that I shouldn't explore any suspicious links, I texted her saying you're violating the guidelines so she's like it's just an initial assessment then she took the job down *sigh*!
I don't think Upwork would do anything about it because a scam can be anyone, you can be a good client today but a scam tomorrow.

If they're immediately asking you to communicate off-platform, it's not a good client (they are violating the rules of the platform). Just treat such "invites" as dealbreakers and flag the job.

Sometimes links are legitimate and useful, whether placed in the job posting or sent to a freelancer who replies. They can link to a website that needs to be worked on, for example, so the freelancer can see if they can do the job. But the OP, a new freelancer, being asked to "fill a form before continuing to the next step" - that's a sure sign of a scam.
