Which Pakistani bank is best for withdrawals to Foreign Currency USD account?

To freelancers who withdraw USD to their Foreign Currency USD accounts, I have questions:-

1. What did you present as Proof of Income while opening your bank account? Did you show Certificate of Earnings that is downloaded from Upwork?
2. Does bank also charge any fee? I know Upwork charges $30 for Wire Transfer

I have read few threads saying that SCB has disallowed receiving of remittances into USD accounts, but those were old threads, I want know the situation of 2020
