Why does my GTBank withdrawal keep failing?

Every time I try withdrawing my funds directly into my Gtbank Nigerian account, it fails. Am I getting anything wrong?

This error occurs when I tried to withdraw money direct to my local bank.

I am withdrawing my money for the first time to my local bank.. but I have deposited some money before.. without any issue.. but now while withdrawing I am facing this issue..
