Hello .

My name is aftab ali ,

i am full time freelancer ,
i can see medical billing skills ,
But i can't find medical billing job .
i see try to find medical billig job ?
But i can't see medcail billing catogary

Hi Aftab,

You should be able to select the Other - Accounting & Consulting as your main category and then select the skills (existing skill for the reference: Medical Billing and Coding) according to your expertise. 

You’re also able to create separate Specialized profiles for your expertise. Please check this helpful article for more information about Specialized Profiles available for Freelancers on Upwork.

You have me confused.
Are you a "full time freelancer" or an Upwork employee as stated in your profile?
Do you have a "Master of Business Administration (MBA)" or "bachelor degree (MBA)"?
Are you misrepresenting any of your skills in your profile? That would be a ToS violation.
