"This field is required" whenever i try to log in

In the last couple of weeks, whenever I tried to log in, and after having entered user name and password, I would receive an error report like this: this field is required.

I just cannot log in anymore. I had to create a new account in order to get in and seek help. Have anyone encountered the same problem? Thank you in advance for your kindly sharing your experience.
