where to find the W-8BEN Form?

I registered today as a freelancer and would like to complete the W-8BEN Form part but I don't understand where I can find the form and download it. 

I would highly appreciate your help!

To complete the W-8BEN form, please go to Settings> Tax Information, and kindly provide us with your legal name and address. Thank you.
