If you haven't seen the 40 pages, it is best with an hourly contract.
If you have an idea how long it will take you, you can go for a fixed rate.
Before starting any work make yourself more familiar with Upwork and how everything works.

Billing is simple: Use an hourly contract at your profile-posted rate.
The client can evaluate your work after an hour. Or even after 30 minutes. If you are producing work that has the quality she needs, and at a value she appreciates, she can continue to let you work on the project.
This is fair for both sides.
There is no way to provide a fixed-price contract bid unless you see all the input files first. Who knows what could be hidden among the files?
