Issues with W-8BEN form

Hi there, I have found a W-8BEN form and I've filled it in. However, I'm a freelancer and also work full-time. So I'm not sure if I have filled it out correctly. I also don't have a legal name of my business - do I need to set up a LTD company with Companies House? How do I submit my W-8BEN to you for review / assistance?
