Payment Inactive for Internationals living in the US

Hi,

 

My Getting Paid appears inactive and asked me to complete a W9 Form.

 

I am an international using a working visa and is currenly residing in the US. If I put my US address on, am I suppose to choose US person or non-US person? Is that why my payment is inactive? Thank you so much!
