If you are referring to hourly contracts, you will need to use the Upwork time tracker app to log your time. For more information, please check this article in our help center. The Time Doctor app is not a scam.
There is no rule against tracking your time with Time Doctor in addition to using the Upwork time-tracker.

Time Doctor may indeed do that.
Upwork has no control over how Time Doctor works.
An Upwork client may ask a freelancer to use Time Doctor. An Upwork freelancer is not required to do so, however.
If a client asks me to use Time Doctor then I may say:
"No thank you. I don't do that. I wish you well in finding a freelancer who meets your needs for this project."
Or
"Yes, I will be happy to use Time Doctor while I work on your project."
Or
"I would like to work on your project, but I would only do so while using the Upwork time-tracker instead of Time Doctor. If that works for you, then I will accept the contract."
