Upwork name and Bank account name Mismatch

Hello, Greetings.

I am new to upwork.

With Payment verification Process I got mail of 'payment account does not match your name of upwork account'.

Here my bank name in account statement: **Edited for Community Guidelines**

Here is my Upwork Account name: **Edited for Community Guidelines**

In Upwork Account Name they are only asking me first name and last name, my first name is Jaimin and last name is **Edited for Community Guidelines**

In Bank Account statement Middle name is added so it's **Edited for Community Guidelines**

So how can I figure it out? I could not find any change!
