W-9 - Do I need to register with the state?

Hello! I am new to Upwork and just accepted my first job! 

I am going to fill out the W-9 (as a US Based Person); however, I do not know if I need to register with my state as a business in order for things to be legitimate? 

Also, do I then need to open a business checking account? 

Any help would be appreciated! Thank you,

You are new.  Just fill out the W-9.  Your legal name is your business name. Don't worry about the state unless and until you decided to form an LLC or some such. You can open a business account if your wish - I've never felt the need, however.
