bank account verification problem

I was trying to setup direct deposits added to my personal cheking, but I entered the amounts in wrong order. now the bank account is "Inactive" and won't let me try again. all details (routing number and account number) are correct, and I did receive the two small deposits. I just need to be able to do the verification step again.

tried remove and add the account again, but no thing changed. also failed to create a support ticket

Hi Ahmed,

I checked your account and you will need to fill out our W9 form to proceed further. Once you`ve done this I'll check your account if further steps are needed or you will just need to allow 3 days for the verification process to be completed. Thank you.

I entered the details for `W-8BEN` there, is there some thing else to do?
am I filing the wrong form?
