Certificate of Earnings concern

Hi, can anyone help me it's been a quite some time since I already corrected my real name but still my certificate of earnings hasn't changed. Just wondering where I can submit all docs needed as proof so I can request a copy to be submitted for my housing loan. Please help asap. My Full name is Oliva Coca Sorela, and my certificate of earnings is still Oliva Estrella.
