I would like to confirm that my name change (single-married name) is approved.

Also, I would like to know if my tax information (TIN ID and W-8BEN) has already been reviewed and approved, as well.

I did not receive any email confirming or denying my submission so I would like to ask about it to make sure that everything is all right.
