W-9 tax form:

Help please.   No matter what I do, I can't send the W-9 Tax info.    The  the date area stays shaded.   When I press save.. I always get this same message.    Any Ideas what might be causing the hangup?   I need to get the W-9 submitted so  UW won't withhold a portion of 1st earnings.

Hi Scott,

Thank you for your message. If you are referring to the Date box next to the Signature at the bottom, that will always show the current date and cannot be edited. Could you please add all the required information and click the Save button?

Thanks.  I after many tries I finally found the problem.    One must SCROLL down all the disclaimer W9 information inside the box and not just scroll down to the bottom of the page as I was doing, and then click save.
