Completed Project not showing on agency profile

I have 2 problems:
1- The project I completed is not showing on agency profile. When I goes to agency work history, I am not able to see anything there. Althought I got the contract via agency and finished the job.

2. Whenever I tries to submit client feedback, submit button greyed out and stays there.
I have tried via agency as well as individual account.
