Account is on Hold - verify your identity

Recentlly i received account suspencion, then received an email to fill the form within 4 days to avoid permenant suspention. Then i decied to fill all the information.

But in my local ID card there is no address written(and in passport as well, in passport only in armenian and in 3rd page so i can not attach second image).

Then in bank statement there is no address shown usually in my country, only in utility bills, but utility bills are all in armenian, so i had to upload bank statement, in which not written my address and my name is in armenian, you can compare names between ID card and bank statement if that is a problem. Please contact me if something is wrong.
