Payment account with company name verification

I have recieved email from UpWork asking me to provide documents with company name and my name:

• Articles of Incorporation showing the company name and your name as an authorized agent
• Business license showing the company name and your name as owner or partner
• A current statement (dated within 90 days) showing the company name and payment provider contact details

But when I clicked the link in the email there was only form to upload passport image and utility bill image. Nothing regarding the company.

Also after upload there was error: "There was an error loading this page: error [709a14fbeeff67c0-SEA, 709a14fbeeff67c0-SEA]. Contact Upwork Support for more information."

Could you please open a support ticket for me so that I could attach all my files there?
