Bug report (Disabling Authenticator app code)

Where: 

Profile icon => Settings => Password & security => Two-step verification options => Authenticator app code

Bugs:

1. Authenticator verification => Authenticator App modal dialog "Delete" button is misaligned, so it looks like there is only "Cancel" button

2. Security question not asked

3. "Save" button can be enabled and clicked (again without sequrity question)
