as top rated user I would like to remove negative private feedback

Hello, as a top rated user I know I can ask to remove a feedback from my history, but I cant open a request in my support.upwork page, I dont know why there is only emply page without any fields to fill.

My other question is if the public feedback is positive but only private one is negative and if the whole would be removed does it will affect positively my JSS ?

Or it only affect the JSS through public feedback that could be removed ?
