I need some idea of criteria for feedback to client, especially related to the "STAR" system. Is there any?

I don't know of any official criteria. 

You should be honest in order to warn other freelancers of potential problems. But also keep in mind that prospective clients may look at your rating of this client when assessing whether they want to work with you. (I'm not sure how many clients dig that deep when evaluating freelancers.)
