Withdrawals being rejected

For some reason, my fund withdrawals keep being rejected. Happened last month and now this too. I've tried withdrawing I twice today but both transactions didn't go through and my payment method got disabled each time? What's up with that Upwork? Also - interesting enough I keep getting charged for these failed withdrawals.
