Locked Out of Account

I got logged out of both desktop and mobile app (on my main account) and now cannot verify my account. I tried to use the form, but I never withdrew / made any transaction and do not remember the card number as I misplaced the card. I have both the email and password for the account yet cannot log in because of the two-factor verification.
