issue with verification

Hi  i am  loosing  my mind   here

I am  having issue  with  verification 

banner comes up, when  i am clicking through it  comes up for  me attached image with incorrect name that  i never added  ,and no idea why  correct name is not coming up and i am unable to change it., conctacted upwork ticket was created gor reply that verification cant  be done , and unable to get any  further responses,did anyone had  this situation before , how  to  about  it, is there way  to  get some direct contact with some agen on this?
